What Type of Documents We May Require and How to Provide Them to Us

At Claimsplus Lawyers, we understand that not everyone has immediate access to a printer or scanner. Our process is designed to cater to the tech-savvy and those who prefer traditional methods.

Documents We Might Require:

Before delving into your insurance entitlements investigation, certain superannuation funds may request:

  1. Authorities: Authorisation letters or forms that allow us to communicate with superannuation funds, the ATO, and other relevant parties to obtain necessary information on your behalf.
  2. Personal Identification: Copy of your passport, driver's license, or birth certificate.
  3. Certified ID: Certified copies of identification documents.
  4. Superannuation Statements: Latest statements from your superannuation fund.
  5. MyGov Super Report: Your comprehensive superannuation report from MyGov.
  6. Medical Records: Reports and certificates from your healthcare providers.
  7. Employment Records: Pay slips, employment contracts, or termination notices.
  8. Insurance Policies and PDS (Product Disclosure Statement): Relevant insurance documents, including TPD or IP policy details and their PDS.

Note: Some funds may accept copies or scans, while others require original documents.
