Documents We Might Require:
Before delving into your insurance entitlements investigation, certain superannuation funds may request:
- Authorities: Authorisation letters or forms that allow us to communicate with superannuation funds, the ATO, and other relevant parties to obtain necessary information on your behalf.
- Personal Identification: Copy of your passport, driver's license, or birth certificate.
- Certified ID: Certified copies of identification documents.
- Superannuation Statements: Latest statements from your superannuation fund.
- MyGov Super Report: Your comprehensive superannuation report from MyGov.
- Medical Records: Reports and certificates from your healthcare providers.
- Employment Records: Pay slips, employment contracts, or termination notices.
- Insurance Policies and PDS (Product Disclosure Statement): Relevant insurance documents, including TPD or IP policy details and their PDS.
Note: Some funds may accept copies or scans, while others require original documents.
How to Send Documents to Us
